Charting in Health Care and Social Services

Charting, also known as documentation, is the process of recording information about a patient's health and care in healthcare settings. This includes medical history, treatment, progress, and interactions with healthcare professionals. In social services, charting is used to document interactions with clients, assessments, interventions, and service delivery.

Applied Charting

One of the primary efforts we engage in is documentation.  We call our overall effort as a consumer of health and human services ProSumption.  Another effort that we enjoy is External Quality Review.  Our charting is a more orderly approach to journaling.  For us ProSuming is engaging proactively with our healthcare, this includes preparing for our appointments and maintaining health related partnerships.

Proactive

(Of a person, policy, or action) creating or controlling a situation by causing something to happen rather than responding to it after it has happened.

Consumer

A simple definition of a consumer is someone who buys and uses goods or services for their personal use, not for resale or business purposes. They are essentially the final users of products and services.

ProSumption

Prosuming, coined by Alvin Toffler, describes a situation where consumers also act as producers, actively participating in the creation or customization of products or services. This means consumers are not simply passive recipients but are engaged in the production process, often sharing their experiences and ideas online. Development – Housing Solutions Bundle (HSB)

Housing Advisory Board Information Technology (HABIT) and Housing Advisory Board (HAB) first appear in our database on July 26, 2018.  A Housing Solutions Bundle (HSB) first appears in our database on June 29, 2022.  One of the key features of a HSB is Roommate Matching in a Rent a Shared Room environment.  On January 23, 2019 we chartered our: Free! Room - Dynamic Log - Primarily 2019 to 2020

One of the features that make housing a “Complex Issue” is the dynamics.  With turnover of either partner in a shared room the consumer has to start again assessing and adjusting.  At times the consumer may think that difficulty with living arrangements is their own problem.  Unfortunately, in some instances it is the consumer’s problem rather than their roommate.  Just like any close relationship it takes time and effort to be a Great Roommate.  It is never too late to figure things about living arrangements out.

Define Congruence

When two or more things are congruent to each other it means that they are compatible, or agree with each other.

Service Battery

In all cases the landlord needs to be evaluated.  Most landlords lack a tendency to evaluate themselves.  With a Housing Solutions Bundle the Advisory (Element) will evaluate landlords.  As does matching with roommates the consumer needs to match well with the landlord.

Behaviorally Challenged Consumer and Dying to Young (DTY)

How is it SleepAbility?

SleepAbility is a term we coined for the host (consumer) controlled factors involving sleep.  In earlier studies we made the mistake of defining SleepAbility to include environmental factors.

Lesson Learned from SleepAbility Studies

We use the room to sleep, maintain our home office, and create things.  Quiet time is basically 8pm to 8am.  We use headphones as does our current (2025) roommate JB.  To give our roommate time alone and privacy we go out into the community and work in our car.  We also go on respites that we call Overnight Adventures.  We are living in Harvest Retirement Community.  The use of the facility is to get assistance with day to day tasks.  Actually they force assistance with some tasks including: Cooking, food and medication.

Transferring and Mobility

Transferring and mobility refers to the process of safely moving a person from one place or surface to another, which can involve various levels of assistance and assistive devices. This includes bed mobility (moving within the bed), transfers between surfaces (like a bed and chair), and ambulation (walking). Proper techniques and assistive devices are crucial for safe and effective transfers, especially for individuals with mobility limitations.

Discharge - Skilled Nursing

A nursing home can't generally discharge or transfer a resident unless specific legal reasons apply, including the resident's condition worsening to a point where the facility can't provide adequate care, the resident's health improving to the point where nursing home care is no longer necessary, or if the resident's behavior endangers others. If a resident is facing a transfer or discharge, they have the right to appeal the decision and the facility must provide written notice with specific details about the reason for the transfer, the effective date, the new location, and appeal procedures.

Inability to Transfer Self

Inability to transfer, also known as impaired mobility or difficulty moving from one place to another, can be a significant challenge for individuals, particularly those with mobility limitations or injuries. This can impact their ability to perform everyday tasks and can require assistive devices or help from others to complete transfers safely and effectively.

What Criteria Must Be Met to Place Someone in Skilled Nursing?

To be eligible for skilled nursing care, individuals generally need to have complex medical needs that cannot be adequately addressed at home…This often includes requiring assistance with daily activities, post-hospitalization care, or needing ongoing medical treatment and supervision. Medicare and other insurance plans typically have specific criteria for coverage, often requiring a qualifying hospital stay and demonstrating a need for a higher level of care.

Ethical Issues and Poor Matches

A poor matching roommate can cause many problems including: Crises, Intentional Harm, Bullying, Torture, Etc.  When the challenges exceed ability to cope a crisis ensues.  Quite often consumers call “911”.

Although sleep may not be pertinent to every visit across every subspecialty, a routine physician inquiry about sleep would be an important part of a typical office visit for many. Sleep problems can predispose individuals to many medical conditions, and, conversely, medical disorders can lead to sleep disturbance.

Notes on Karl

Possibly using stimulants, his TV service was cut, Ranting and Raving day and night, got into it with his father, threatened to involve police, taken away by ambulance.  Because of Karl, Buster did not sleep well for a week or so.  Buster’s health deteriorated.  The facility was not a good match.  Many consumers’ agree that the facility (Chapman) is sub-par.

An unreasonable accommodation in housing and living arrangements is a change or adjustment that would impose an undue hardship on the landlord, making it significantly difficult or expensive to implement.  Examples include changing rooms too often, lowering behavior standards, or moving out and wanting to come back.  An undue hardship can also be caused by the cost, the size of the operation, or the nature of their operation.  In addition asking for help (intervention) can be unreasonable.  If the consumer requires numerous interventions this might indicate they may be better off moving.

Mental Pain and Suffering
