Charting in Health Care and Social Services

Charting, also known as documentation, is the process of recording information about a patient's health and care in healthcare settings. This includes medical history, treatment, progress, and interactions with healthcare professionals. In social services, charting is used to document interactions with clients, assessments, interventions, and service delivery.

Applied Charting

One of the primary efforts we engage in is documentation.  We call our overall effort as a consumer of health and human services ProSumption.  Another effort that we enjoy is External Quality Review.  Our charting is a more orderly approach to journaling.  For us ProSuming is engaging proactively with our healthcare, this includes preparing for our appointments and maintaining health related partnerships.

Proactive

(Of a person, policy, or action) creating or controlling a situation by causing something to happen rather than responding to it after it has happened.

Consumer

A simple definition of a consumer is someone who buys and uses goods or services for their personal use, not for resale or business purposes. They are essentially the final users of products and services.

ProSumption

Prosuming, coined by Alvin Toffler, describes a situation where consumers also act as producers, actively participating in the creation or customization of products or services. This means consumers are not simply passive recipients but are engaged in the production process, often sharing their experiences and ideas online. How is it SleepAbility?

We use the room to sleep, maintain our home office, and create things.  Quiet time is basically 8pm to 8am.  We use headphones as does our current (2025) roommate JB.  To give our roommate time alone and privacy we go out into the community and work in our car.  We also go on respites that we call Overnight Adventures.  We are living in Harvest Retirement Community.  The use of the facility is to get assistance with day to day tasks.  Actually they force assistance with some tasks including: Cooking, food and medication.

Transferring and Mobility

Discharge - Skilled Nursing

Possibly using stimulants, his TV service was cut, Ranting and Raving day and night, got into it with his father, threatened to involve police, taken away by ambulance.  Because of Karl, Buster did not sleep well for a week or so.  Buster’s health deteriorated.  The facility was not a good match.  Many consumers’ agree that the facility (Chapman) is sub-par.

Abstract – Executive Summary

This paper addresses Quality of Life (QOL) in the face of Housing.  In particular we investigate Renting a Shared Room (RASR).  We apply our, Mentalation Solutions Group’s (MSGs) Transparency Principle in that we share real case records.  We have included four (4) assessments associated with QOL including the results for Keith “Buster” Torkelson MS.  All results arise from Buster’s self-scoring.  We include Parity Checks In three (3) of the four (4) assessments.  Parity Checks help us to uncover outlandish results.  Looking back, during the period from 2012-2023 here at MSG we scored many assessments too favorably.  In this study we have identified a few assessment items to be higher in Weight.  In the future we will address these High Weight Variables (HWVs).  For us and many others including “Normal-s” Sleep is a HWV for QOL in the context of Housing. 

This material is written for everyone yet we address two Special Populations: Homeless and Criminals.  We hope to facilitate members of both groups find if need be a RASR GoodBed.  We have a project plan to deal with gaps in the Housing Systems: We call our project idea a Housing Solutions Bundle (HSB).  In this study we make a note of Buster’s Advance Directives.  One of Buster’s most important Supports (Natural Supports) is his family.  About us utilizing Hyperlinks in this report.  We chose to not change or truncate Links by creating a Hyperlink.  Hyperlinks can be unreliable.  We share a bit on our Data Bases.  We hope our “W” Work Flash Drive survives us.  Last, we address diagnosis.  We posit or postulate that Crime is a disease.  Which leaves us to believe that:  Behavioral Health Issues + Substance Use Issues + Crime and Criminality = Triple Diagnosis.

Contexts

Presenting Problem

Below are a few of the Special Populations (SPs) that need superior service in order to stop the revolving door or Stop the Cycle.  Those in one or more SP / SPs need to be able to pick up the phone or Smartphone and call one number to serve the majority of their Individual Housing Needs (IHNs).  There are many common hurdles on the way to a GoodBed.  In addition, there are many individual centered hurdles on the way to a GoodBed.

Introducing a Housing Solutions Bundle (HSB)

A Housing Solutions Bundle (HSB) is a Suite of efforts all centered about placing or helping place the disadvantaged including the Homeless and Criminals.  Our HSB) includes: A Housing Helpline, a Housing Work Group, a Housing Advisory Board, Call Center Infrastructure, and Project Metrics.  We submitted our HSB Mental Health Services Act (MHSA) Innovations Project Idea last March 29, 2023.  Amongst other favorable outcomes the benefactors of our HSB will very likely actualize an improved Quality of Life (QOL).

Some Variables about QOL

Below is a table of the results from our Brainstorming.  Here are some variables associated with Housing and Moving.  HW indicates the variable is a High Weight Variable (HWV).  All of the remainder Items are Medium Weight Variables (MWVs).  In the future we will re-weight some HWVs variables.  HWVs we will address HWVs three times in the assessment, whereas we will address MWVs twice in the assessment.

As consumers of Behavioral Health Services (BHS) many of us are dependent on a RASR, medication, Medicare, Medi-Cal, Disability Insurance or Supplemental Security Income, Social Security,  etc.

Intimacy

In many cases it is best to give up being intimate and having sex in the RASR.

Gradually we have had to give up property, for example furniture.

We don’t address Spirit as much as we did when we were with our Most Significant Other Ever (MSOE) – “Nonphysical part of a person which is the seat of emotion and character, the soul” – We have a separate spiritual study called Deal with God (DWG).

Aside - Family and Quality of Life

Your family is one of your Natural Supports.  Families can Rock.  They can make a huge difference in a disadvantaged person’s life.  That is Buster there on the right.  You might have to Resource Level so you do not make too much of a burden on one family member.  In addition you probably should not rely on a family member that is discordant.  They themselves may be stuck in a cycle of their own.

Here are some findings to share with our Housing Advisory Board HAB-QOL.  We have settled on about ten (10) approaches to assessing for QOL.  A powerful approach when evaluating your QOL is the ability to feel Satisfied.  If you cannot feel Satisfied your QOL will suffer.

When I talk with people I try to learn as much as possible about local Housing for the disadvantaged including the homeless and criminals.  In the context of a Housing Solutions Bundle I evaluate housing needs in the OC.  The Behavioral Health Services Act (BHSA) puts more emphasis on housing than did/does the Mental Health Services Act (MHSA).
